Nurses in community-based healthcare visit their patients at home and offer rehabilitation and physiotherapy services (Cohen & Cesta, 2005). Community-based healthcare models are more costly to operate than hospital-based healthcare models. There are more patients in the community-based models because of their low fees as compared to hospitals.

Most patients that frequent hospitals do so when there have complicated ailments that cannot be taken care of at the community-based models. There is specialized treatment in hospital-based models than in community-based models and this is the reason hospital-based clinics serve fewer patients than community-based clinics. Focus on hospital-based services requires counseling, assessment, and alternative and complementary treatment that is higher when compared to community-based models (Cohen & Cesta, 2005). The hospitals have more specialized specialists with the ability to execute the interventions. When we compare patients in community-based models with those in hospital-based models, they have fewer caregivers, and thus it is necessary to provide them with counseling and assessment teams (Cohen & Cesta, 2005).

Community-based healthcare provides home care especially for the elderly and those who need interventions have to be referred to hospital-based healthcare services. Nurses play the role of providing community-based healthcare services whereas doctors take responsibility for hospital-based healthcare services (Thornicroft & Tansella, 2004). Due to this fact, services at community-based health services are not as satisfactory as hospital-based services. Nonetheless, the nurses play a vital role in ensuring that those who enroll for services at the community-based services get the necessary services and for a case, they cannot solve, they refer them to the hospital-based healthcare.

There have been challenges facing community-based healthcare due to the lack of the required equipment to fulfill the needs of the patients (Thornicroft & Tansella, 2004). It is because of this fact that most patients prefer to be served at hospital-based healthcare though services at these facilities are slower than in community-based healthcare. The models impact nursing care by offering them a platform where they can exercise their roles and help the healthcare sector in achieving its main goals of offering health services to the needy (Thornicroft & Tansella, 2004). Health budgets are planned to use the models and thus enough planning is done to provide the services.

Conclusion

Case management models in care delivery play a vital role in ensuring that the clients get satisfactory services as well as in ensuring the practitioners work in an ample environment. The case management models also facilitate improved health services and the prevention of illnesses in the community. Though services in hospital and community-based healthcare may differ, their aim is universal. They strive to ensure that illnesses are prevented and treated.
